Children and families experience many transitions, large and small over the years. There are many things to consider and plan for as any teen transitions from youth to young adulthood - continuing their education, getting a job, moving out, to name just a few.
For young people who have chronic health conditions, health and wellness is a critical component in transitioning to adulthood. Planning for the move from the child-centered health care system to the adult-centered health care system is crucial, but with everything else going on during this time, things can often feel overwhelming for teens or young adults and their parents.
As a youth, parent, or medical provider you may be caught up in day-to-day details, but this doesn't mean that planning for the future should be postponed. With your child’s medical providers, you can work as a team to look to that future.
Join us! These day-long parent and youth conferences will offer information and ready-to-use planning tools and resources for youth, families, and providers to make the health care transition process easier and less overwhelming.
